Continuing Care Team


An exciting opportunity has arisen to join the CYP continuing care within the Whittington NHS for  band 3 Community Support Workers.

The children and young people’s continuing care team provide care to children or young people, who are in receipt of a Continuing care package within the London Boroughs of Islington and Camden.

The successful applicant will join a dynamic team of Children’s Nurses and other Community Support Workers who provide shifts for children and young people in their homes or educational community settings. These children and young people have complex health  care needs.

You will be a highly motivated and enthusiastic individual with a willingness to complete a robust and in-depth training programme which is tailored  around the health needs of the child or young person.  You will receive ongoing training and support throughout your time with the team.

This is a full time post with 150 hours a month being completed, however part time hours for the right candidate would be considered.   Shift patterns are predominantly nights with some day time activities with team days and training.  The vacancy is a fixed term period of 12 month, with the possibility of becoming permanent if the service expands.

This post involves working with children and/or vulnerable adults and so the successful candidate will be required to apply for Disclosure.
