An exciting opportunity has arisen to join our Rapid Response/Virtual Ward team within Whittington Health in North London. The service is at the forefront of integrated care – an innovative and transformational team that best serves our local community.

The Rapid Response Virtual Ward Team comprises of GPs, nurses, therapists and carers. As part of one of the largest ambulatory care units in the country, the service enables patients with sub-acute conditions to recover in their own homes, preventing unnecessary hospital admissions and reducing length of hospital stay.

 

We are looking for an experienced RN with a passion for high quality nursing care and also has a background in Acute medicine, Emergency Care or Community Nursing. The post holder will provide acute nursing interventions in patients’ own homes and will ensure that patients are referred to appropriate community services to keep them at home safely. The vision of the service and is key to bridging the gap between hospital and community - bringing care closer to home.  

 

The role will give the post holder the chance to work with colleagues from acute specialities, the emergency department, and primary care as well as community services as part of an Integrated Care Organisation (ICO). The successful applicant will have the opportunity to undertake a Masters pathway as part of the role.
