Whittington Health has been delivering expert community services to UCLH patients.

The service supports transfers from inpatient wards to community settings for the remainder of their care pathway.

The post holder will work closely with the UCLH Integrated Discharge Service, Inpatients wards of various specialities as well as community teams.

The role will provide a unique opportunity to be part of embedding exiting service to provide truly integrated care for patients across acute and community settings; developing and expanding the services to provide the best care possible for our patients.

 This is an exciting and dynamic place work, and a fantastic chance to join a dedicated and growing multidisciplinary team on their journey.
