Role: Lead Nurse or Advanced Clinical Practitioner (ACP) in the Integrated Community Ageing Team (ICAT)

Whittington Health are looking for an enthusiastic experienced nurse or an Advanced Clinical Practitioner (ACP) who is passionate about care of frail, older people to join ICAT. ICAT was developed in 2014, led by a consultant geriatrician, Dr Ruth Law and a GP with a Specialist Interest (GPwSI), Dr Philly O’Riordan to provide high quality care to frail elderly within Islington. The team also includes senior nurses, senior pharmacists, senior physiotherapists, a senior occupational therapist and a therapy technician as well as a patient pathway co-ordinator. We are an engaged and committed multi-disciplinary team that strives to provide high quality, integrated patient care.

We are looking for an exceptional nurse or ACP with excellent assessment skills, as well as knowledge and experience of caring for frail older people with long term conditions and to help lead the ICAT team. This nurse/ACP will be expected to carry out community based comprehensive geriatric assessments, usually within the patient’s own home in Islington. ICAT also run clinics within Ambulatory Care at Whittington hospital when further investigations might be required. We aim to provide rapid access, multidisciplinary support for patients and maximise their time spent at home. Our team have frequent MDT meetings, led by our consultant or GPwSI.

This is an exciting opportunity for a nurse who is passionate about providing high quality, integrated care for older people. The nurse/ACP will have the opportunity to work with colleagues across the primary and secondary care interface. They will also require good leadership skills as they will have the opportunity to input into the strategic development of the service.
