CHC Clinical Team Lead Haringey, Band 8a
An exciting opportunity has arisen in Whittington Health for a motivated and enthusiastic individual to lead the Clinical Continuing Healthcare team for patients who are registered with a Haringey GP.
The successful candidate would be expected to lead and inspire a team to deliver of high quality assessment, reviews and patient care in accordance to the National CHC Framework and best clinical practice and guidelines.
You will be responsible for ensuring all aspects of the day to day operational delivery of the Continuing Healthcare service and will be working alongside senior management, commissioners, partners working in the other areas and multi-agency providers, families and carers.
Whittington Health is an integrated care organisation (ICO) providing closely integrated working across the hospital and community sites. The Continuing Healthcare team is based in Islington.
The trust provides assistance with season tickets. However there is excellent local transport in this part of London.
This opportunity is open to Registered Nurses at Band 7 or above with CHC and management experience.
